Below are links to forms downloadable from my website which relate to particular potential legal representation. Each includes a
client interview form, which inquires as to information I need necessary to prepare paperwork and to represent you, and an
agreement regarding attorney's fees and costs, which addresses how those are to be paid. You may download, print and complete
these forms if retaining me by mail and, if retaining me in person, you may elect to complete them in advance and bring them with
you as opposed to filling them out while sitting at my office.
New Domestic Case Interview Form
applies to new divorce or civil
paternity actions that you seek to
file or which are now happening in
Court but which have not been
resolved through a final order.
Post Domestic Case Interview Form
applies to contempt applications,
motions to modify or enforce, or
other motions that you seek to file or
which are not happening in Court
and where a final decree or order
was previously entered in a Court.
Uncontested Divorce Interview
Form applies to instances where I
am willing to take the case for a
lower amount and flat fee up front
unless and until certain conditions
occur, at which point the case begins
to be treated like other cases. The fee
agreement is self-explanatory. You
must at least talk to me by phone to
get my approval of this lower fee.
Guardianship Interview Form is
used if you seek a guardianship and
no case has yet been filed or if you
wish for me to enter a case that has
already been filed, whether to seek a
guardianship or try to terminate an
existing guardianship. This form
may also be used for stepparent or
third-party adoptions since the
necessary information is the same.
